Informed consent & your choices

Your care is a shared decision. Ask questions, tell us your concerns, and choose what you are comfortable doing.

Before an examination or treatment

Dr. Lenny will discuss what is proposed, the goals, expected benefits, meaningful risks, reasonable alternatives and what may happen if you wait or decline. The discussion should reflect your health, priorities and the specific care being offered. Results cannot be guaranteed.

Exercise and hands-on care

Possible effects include soreness, fatigue, temporary symptom increases, dizziness or loss of balance; specific procedures may have other risks. Ask about the risks that apply to you. Follow only the exercises, assistance, equipment and dose assigned. Tell your clinician about symptoms or changes in your health. Ask before changing an exercise you do not understand.

Your choices

You may refuse a particular examination or treatment, ask for another option, pause, or withdraw permission. You may ask about touch, positioning, clothing or draping and who will participate. Tell us if you need an interpreter, another communication method or help from an authorized representative. A new plan or material change calls for another discussion.

How consent is documented

Your clinician documents your decision and the discussion in your clinical record. Reading this page, opening a program or downloading instructions does not sign a consent form or show that the discussion took place. Complete the secure intake process or discuss the printable form with your clinician before care.

Other permissions

Permission for photographs, videos, testimonials or advertising is optional and separate from treatment. Recording or AI-assisted documentation should be explained separately before use. Privacy acknowledgments and financial agreements serve different purposes. Public exercise resources provide general information and do not create a treatment relationship.
